Students and employees can receive free medication lock boxes at the Student Health Center (SHC) pharmacy. Just stop by the pharmacy, complete a short request form, and get your lock box at that time.
Request forms
Option 1 – Hard copy: Ask the pharmacy staff for a hard copy to complete when you visit the pharmacy.
Option 2 – Online survey: Complete in advance and let the pharmacy staff know that you’ve submitted the survey. To access the survey, click here.
